Personal History Questionnaire

To address some of the rumors about the PHQ in the hiring process. Command has removed it from the initial packet, however the background is still a large part and many of the questions were repetitive to the PHQ. Command is trying to save resources to better address those that make it to the Background phase rather than those that can't even pass enough to get to that stage. This is not a reduction of quality or risk that new applicants are not getting vetted.
